Outcome Assessments (ReviewWave)

1. Select the ordered outcome assessment questionnaire on the iPad.
2. Enter the patient’s name and date
3. After patient completes the questionnaire, log in to the ReviewWave on the computer
4. Click on forms, show forms, click on the clipboard in the row of the questionnaire, click on the questionnaire
5. Add up answer numbers for each question to get the score.
6. Enter score in the patient’s note: Open the patient’s note; in the assessment section, click Outcome Assessment. Choose the correct questionnaire, then next. Type in Score; Save Note.
7. Delete questionnaire in in Review Wave.
