- BCBS/Blue Plus/Humana/Medicare/Medica/Aetna: Log into essentials.availity.com. Under Patient Registration, click on Eligibility & Benefits Inquiry. Choose the correct payer, enter the patient’s information and click submit.
- Cigna: Log into cignaforhcp.cigna.com. Click on the Patients drop down menu then Search Patients. Enter the patient’s information and click search.
- HealthPartners: Log into healthpartners.com/provider-public/login. Click on the Applications drop down menu, then Eligibility Inquiry. Click I understand, enter the patient’s information and click search.
- Medical Assistance: Log into mn-its.dhs.state.mn.us/gatewayweb/login. Click on the MN-ITS drop down menu, then Eligibility Request. Enter the patient’s information and click submit.
- PrimeWest: Go to primewest.org/login; click on the Provider Web Portal link to login. Click on the Eligibility & Benefits tab, enter the patient’s information and click search.
- South Country Health Alliance: Log into provider.mnscha.org. Click Proceed in the pop up box. Click on the Eligibility and Claims drop down tab, choose Eligibility and Claims. Enter the patient’s information and click search.
- UCare: Log in to ccmiproviders.lmhealthcare.com/login.aspx. Use the Member Search tab to enter the patient’s member ID and first name (or use the alphabet shortcut buttons by the patient’s last name if they’ve had a recent claim). Click to fill in the blue circle where it says Select next to the patient’s name, then click Eligibility.
- UnitedHealthCare/AARP: Log into uhcprovider.com using the OneHealthCare ID log in information. Click on the Eligibility tab on the left side of the page, and choose the correct payer on the middle-right top of the page. Enter the patient’s information and click verify eligibility.
- UMR: Log into provider.umr.com using the OneHealthCare ID log in information. Enter the patient’s ID at the bottom of the page and click search. Next to the patient’s name, choose Benefits under the View drop down box and click search.
- Other (Allied, Avera, Bywater, Sanford): Call phone number on the back of the patient’s card for benefit details.
