Adding Outcome Assessments as a Web Page

  1. Open Google Chrome. Click on the three vertical dots on the upper right corner of the screen (right below the x to close the window). Click on New Incognito window.
  2. Log in to redrockchiro.com/wp-admin/
  3. In the left side menu, hover on Pages then click Add New Page.
  4. Click into the Add title area and type the name of the form ex. Ankle Outcome Form.
  5. Click on Launch Page Builder Tool (blue box). It will take a bit to load.
  6. In the right side menu, in the Modules section, click on the magnifying glass and type embed.
  7. Click and drag the video icon from the menu into the content box.
  8. Under the drop down menu, change it from Media Library to Embed.
  9. In ReviewWave, hover over Forms, then click on Show Forms. Click on the form you want to open, then click Copy Embed Code (blue box).
  10. Back in the new web page, click on the long skinny gray bar (line 1) then paste the code. Click Save.
  11. Click Done at the upper right corner of the screen then Publish.
  12. At the upper left corner of the screen hover over Red Rock Chiropractic Center then click on Dashboard.
  13. In the web pages section of the Dashboard, find the new webpage and click Quick Edit to change the title tag and meta description. In general, you can use “Compassionate Care at Red Rock Chiropractic Center” for the title tag and “Discover patient-based care in Lamberton, MN” for the meta description.
  14. Repeat the same process for all the outcome forms.
  15. Add the web page as a short cut on the iPads.
