- Adding patient: Log in to fmlogics.com clinician login; click on Patients tab; click on Create New Patient; add patient information and save. Use patient’s e-mail address for user ID.
- Sending questionnaire: click on Questionnaires tab ; use search bar to find patient; in the Questionnaire drop down menu choose Comprehensive Patient History; to the right click Activate.
