Orthotics Returns

1. Returns can be made within 1 year of purchase but after allowing 45 days for the break-in period. Return authorization forms are in the black file box on the counter behind the front desk. Write the patient’s name and the reason for returning on the form.
2. Call Foot Levelers 800-553-4860 for a return authorization number. Write this at the top of the form.
3. Scan a copy of the form into the patient’s account, then package the old orthotics and include the return form inside. Mail the package to Foot Levelers 518 Pocahontas Ave., Roanoke VA 24012.
